What to Expect on Your First Visit

Our dedicated healthcare professionals are here to listen to your concerns, answer your questions, and develop a personalized treatment plan that meets your unique healthcare needs. Our treatment plans include a multi-modality approach which may include injection therapy or medications. Medications however, will not be prescribed at the initial consultation until your care is established with our clinic and we see you for a follow up visit. We will always discuss the pros and cons of your treatment options as your input, comfort, and understanding are paramount to us.
Preparing for Your First Visit
To help make your first appointment smooth and efficient, please:
- New patients must arrive 30 minutes prior to your appointment time to sign our consent forms and meet with the medical staff. If you would like, you may review these consent forms prior to your visit. Late arrivals will result in your appointment being rescheduled.
- Bring a valid photo ID.
- Bring physical copies of your insurance card(s).
- Compile a list of current medications
- Bring any relevant medical records or recent test results including x-rays or advanced imaging such as MRI or CT scans.
- If you have been seen by a primary care physician, neurologist, neurosurgeon, orthopaedic specialist or another pain clinic, we kindly ask that you contact their office and request your records to be faxed to us at 984-272-3917.
Important Insurance Information
- We accept most major insurance plans. Please review our list of accepted insurance plans or contact our office for insurance-related questions and help you understand your coverage.
- Office visit copays are due at check in. We are considered a specialist office, so please refer to your insurance card for the copay amount that you will be expected to pay.
- For those with insurance plans that are based on a deductible, we will collect the amount of our contract with that plan.
- Any labs such as urine drug screens, are billed as a separate charge to your insurance and may be subject to your deductible.
- You are encouraged to contact the customer service number on the back of your insurance card to verify your coverage and benefits.
- Please note that Midtown Pain and Spine is a division of East Carolina Anesthesia Associates, PLLC. The billing tax ID is 20-1442673 and NPI is 1205996519.
