Midtown Pain and Spine Clinic files insurance as a courtesy to our patients. Please note that some insurance companies require special authorization to either see a specialist or use out of out-of-network benefits. If you have questions regarding your policy, we ask that you contact your insurance prior to your visit. The insurance carrier will have the most up-to-date provider listings including details on narrow networks and how they work.

Below is a list of insurance plans with which we are currently in-network. All other insurances are considered out of network. We will file claims on your behalf and if you have benefits for out-of-network services, your services may be applied toward them, otherwise you will be financially responsible.
